When might a fertility specialist refer a man to an andrologist or a urologist?

Lucknow : An abnormal semen analysis can identify a problem with sperm count, movement or shape, but it may not explain why that problem exists. This distinction is important because some causes of male infertility, including hormonal disorders, varicocele, obstruction or testicular conditions, may require evaluation beyond routine fertility testing, says Dr Shreya Gupta, Fertility Specialist, Birla Fertility & IVF, Lucknow.

Current male infertility guidelines recommend that men with abnormal semen parameters or suspected male infertility undergo evaluation by a male reproductive expert.

When is a specialist referral useful?
A fertility specialist may involve an andrologist or urologist when there is:
●    Persistently low sperm count or poor sperm motility
●    Azoospermia, where no sperm are found in the ejaculate
●    A suspected or diagnosed varicocele, or enlarged veins around the testicle
●    Erectile or ejaculation difficulties
●    Abnormal reproductive hormone levels
●    Previous testicular injury, infection or surgery
●    Small or abnormal testicles on examination
●    A suspected blockage in the male reproductive tract
Referral may also be considered after repeated assisted-reproduction failure or recurrent pregnancy loss when further male evaluation is appropriate.

So, what does each specialist do?
An andrologist focuses specifically on male reproductive health. They may assess sperm production, hormones, testicular function and conditions that could be affecting a man’s fertility.

A urologist specialises in the urinary system as well as the male reproductive system. They may be involved when the evaluation points towards a structural or surgical concern, such as a varicocele, obstruction or certain testicular conditions. The roles can overlap, and not every man will need to see either specialist.

Why would a fertility specialist need another opinion?
Sometimes, a semen analysis tells you what is happening but not why. For example, a persistently low sperm count could have several possible explanations, including hormonal problems, impaired sperm production or a blockage in the reproductive tract. A more detailed examination, hormone testing, imaging or other investigations may help identify the cause.

Finding that cause matters because treatment isn’t always the same. Some men may benefit from treating an underlying medical or structural problem, while others may need assisted reproductive treatment.

Does a referral mean fertility treatment is the next step?
Not necessarily, because the purpose of bringing another specialist into the process is to understand the problem properly before deciding what comes next. Depending on the findings, the options could include medical treatment, a procedure or treatments such as IVF with ICSI.

Male fertility is part of the couple’s fertility assessment, not an afterthought. When a fertility specialist, andrologist or urologist works together where needed, it can help ensure that the underlying problem is identified and the treatment plan is based on the actual cause rather than just the semen report.
