Lucknow : Menstrual cycles are not just a marker of reproductive health; they are the foundation for natural conception. For a 32-year-old woman from Lucknow, this basic rhythm never began. Living with primary amenorrhea caused by hypogonadotropic hypogonadism, along with hypertension and a severely diminished ovarian reserve, she spent more than a decade holding on to the hope of becoming a mother. 

After 11 years of unsuccessful treatments, including a failed IVF cycle where no dominant follicles could be achieved, she and her partner reached Birla Fertility & IVF, Lucknow in search of one final chance.

 That is when Dr. Shreya Gupta, Centre Head & Consultant, Birla Fertility & IVF, Lucknow, led a new approach to her case.

 “This was one of the most complex scenarios in reproductive medicine,” shares Dr. Gupta. “We were working with an extremely low ovarian reserve, which meant every egg mattered. It required persistence, careful protocol design, and emotional support for the patient at every stage. Fertility care in such situations is both science and perseverance.” 

The team created a personalised plan. A modified stimulation protocol was used to overcome ovarian resistance, leading to the retrieval of three oocytes and the development of two viable Day-3 embryos. To optimise the uterine environment, a hysteroscopic uterine cavity enhancement was performed prior to transfer. In a frozen cycle under carefully primed conditions, the embryos implanted successfully in the very first attempt. 

Thereafter, routine antenatal care helped the couple have a healthy pregnancy. With this, the couple was able to achieve a long-standing dream of parenthood, and it proved that science and compassion can lead to results even in the toughest circumstances. 

Dr. Gupta reflects: “This case is a reminder that even women with rare conditions and low ovarian reserve just need the right strategy and approach to achieve pregnancy. It shows how advanced planning, precision, and emotional support can turn possibility into reality.”
